Please complete your intake form as soon as possible after booking. This form is confidential and helps ensure your session is safe and effective.
Important
Appointment requests without a completed intake form will not be accepted. Failure to submit your form in a timely manner may result in a cancellation fee. If a contraindication is identified too close to your appointment time for us to act on it, the cancellation policy will still apply.
